Questionnaire editing

## Prompt

Review this questionnaire: {survey}. Research objective and respondents: {context}. For each problematic question, give the original, issue, revised wording and suitable response type. Look for double-barreled or leading questions, unclear periods, overlapping options and missing ‘not applicable’ choices. Add no questions unrelated to the objective. Suggest an order and brief introduction without promising anonymity unless the conditions support it.

## Template fields

- `{survey}` — Questionnaire. Example input: Questions with answer options
- `{context}` — Objective and respondents. Example input: What needs to be learned, who responds and how answers are collected
